Day 1

Click the buttons below to download and fill up the paperwork, after that one of our doctors will start with you by taking a thorough history. This is where they will collect as much information about you as possible. The more information you can provide (past medical reports, blood work, X-ray/ MRI) the better chance we will have of making an accurate diagnosis, prognosis, and treatment plan. You can speed up this process by faxing your medical records to us prior to your first appointment. Our fax number is 208.343.1953

After the history is complete your physician will perform an in-depth examination including a full spinal assessment, neurological assessment, and orthopaedic assessment. After your physicians review your medical history and complete a thorough examination, further imaging such as X-rays or an MRI may be recommended. For your convenience, we do have an on-site X-ray facility.

Depending on your specific case, your physician may not provide treatment on day 1. This is for your safety. Your physician may require additional time to look through your medical records or to wait for imaging results prior to starting any treatment. Again this is for your safety. Your physician wants to be confident that the treatment that is provided is the correct treatment for your situation.

Day 2

On your second visit to our office your physician will will personally sit down with you and discuss your X-rays, blood work, MRI’s or any other records. This is called a report of findings. At this time your physician will talk about the diagnosis, prognosis, and a treatment plan. Our physicians will make every effort to help you understand what is going on and the treatment options that are available.

After the report of findings you should have an in depth understanding of what the problem is and what is needed to correct it. You can now feel confident that your physician is aware of your specific situation and your treatment is personalized to your unique needs. Day 1 and day 2 typically require 1 hour each. After day 1 and day 2 have been completed, each visits can range from 20-40 minutes.
